Clarity when care gets complicated
When medical needs, mental health concerns, family responsibilities, and unanswered questions pile up, figuring out the next step can become a job of its own.
Charleston Care Collective helps families make sense of the information, identify priorities, and move forward with a practical plan.
Who we’re especially helpful for
Adult children balancing care with work and their own families. Relatives coordinating from a distance. Families managing overlapping medical, mental health, and practical concerns—or trying to move forward when advice conflicts and responsibilities are unclear.
You can hand over the coordination while staying informed and involved.

Why CCC exists
Families often become the connection between professionals, appointments, records, and decisions. That work continues long after an appointment ends. CCC was founded to help carry it.
A note from Nina
“Navigating care for my own family taught me how much work happens between appointments. Someone has to organize the records, follow up on unanswered questions, and keep track of what happens next. I created Charleston Care Collective to help families carry that work—with clear information, practical next steps, and thoughtful follow-through.”
Nina Spotts, Founder
Nina holds a bachelor’s degree in psychology from the College of Charleston and brings experience in operations and complex information management. Her approach combines careful listening with practical organization, helping families turn scattered information into manageable next steps.

From scattered details to next steps
A fictional example
An adult daughter is balancing work and children after her father leaves the hospital. Discharge papers are scattered, follow-up appointments are unclear, and two medication lists do not match.
We organize the documents into a dated timeline, build a shared calendar, and create an action list showing who owns each next step. The conflicting medication information becomes a focused question for his pharmacist or prescriber.
Her working toolkit includes:
• A care timeline with records and family observations identified separately.
• A calendar of appointments and agreed reminders.
• Provider questions and missing-information requests.
• A tracker showing completed tasks, pending responses, and decisions needing her input.
The illustration uses fictional information. Deliverables depend on your agreed scope and authorizations.
What we help you do
• Organize records, correspondence, bills, and appointment notes.
• Build timelines that make patterns and gaps easier to understand.
• Prepare questions for providers and important family conversations.
• Track follow-up, outstanding requests, and responsibilities.
• Coordinate transitions and develop a plan as needs change.
Working alongside your professionals
Our role is care navigation, organization, and coordination. With your permission, we work alongside your existing providers and advisers to connect information and support follow-through. Clinical and legal decisions remain with the appropriate licensed professionals. Independent professionals provide their services under separate agreements with your family.
When information conflicts
We distinguish what the records document, what family members observed, and what still needs clarification. We organize dates and sources, identify discrepancies, and prepare focused questions for the appropriate professional—so concerns can be discussed with useful context.
What working together looks like
1. Bring what you have. Start with the emails, documents, questions, or concerns already on your plate.
2. We make sense of it together. Identify what needs attention, what information is missing, and which decisions come next.
3. Move forward with support. Choose a focused project or ongoing coordination to help put the plan into action.
What you can expect
Before work begins, we agree on priorities, scope, fees, and how we’ll communicate. Your action list shows what is completed, what is awaiting a response, and what needs your decision. We share updates with the people you authorize and revisit priorities as needs change. You remain involved in the decisions that matter.
