Remote Care Coordination & CCM Support

Charleston Care Collective offers remote care coordination and Chronic Care Management (CCM) support for medical practices and established care-management teams. Led by Nina Deuskar Spotts, we help patients and families stay connected between visits.

Support can include patient and caregiver outreach, appointment and referral follow-through, medical records organization, community resource navigation, clinician-directed care plan tasks, and coordination documentation.

Each partnership defines responsibilities, qualifications, approved systems, and escalation procedures. Treating clinicians retain medical decision-making. In a CCM arrangement, the billing practitioner determines eligibility, supervision, and which activities qualify for billing. Coverage and patient cost-sharing depend on the confirmed arrangement.
