When the plan needs follow-through, we help track appointments and unresolved tasks, organize new information, prepare questions, and coordinate with authorized family members and independent professionals.

Your written scope identifies included tasks and hours, deliverables, communication channels, response times, fees, and the review date. Additional work starts only after you approve the scope and fee in writing. Third-party costs are confirmed separately.

CCC provides non-medical, non-clinical support during agreed service hours. It does not provide emergency response or unlimited on-call access. Professional decisions and availability remain with the independent providers.
