Charleston Care Collective
Thoughtful guidance. More room to be family.

Personal care navigation and coordination in Charleston. We help you understand your options, connect with the right professionals, and keep the practical details moving—so you can spend more time being family.
Find your starting point
You don’t need a complete plan before reaching out. Start with what your family needs help carrying.
Something has changed.
You’ve noticed changes over time, but others may see only a good day. We organize observations, dates, messages, and records into a clear timeline—separating documented events from family observations and questions for professionals.
The crisis has passed.
The immediate crisis is over, but the practical work remains. We organize discharge records, outstanding bills, follow-ups, and provider questions, then build a prioritized plan with clear responsibilities and ongoing routines.
You’re carrying the load.
Appointments, paperwork, and competing advice are taking over your days. Hand off agreed coordination tasks, research, and follow-through. We keep next steps visible and authorized family members informed, while you stay involved in decisions.
You can be there for your loved one without having to manage every call, appointment, and care decision alone.
We provide thoughtful, personal attention for your loved one and experienced guidance for your family. Hand off the coordination, research, and follow-through to a professional who helps connect the pieces—while you stay informed and involved.
Does this sound like your family?
A sudden crisis can leave you with urgent questions and no clear place to turn. Families come to us when:
• A loved one is leaving the hospital, and you’re unsure what happens next.
• Changes in memory, mood, or behavior make everyday care harder.
• Living at home may no longer be safe, but the options feel overwhelming.
• Appointments, paperwork, and competing advice are piling up.
• Family members disagree, or the responsibility keeps falling on you.
• You’re balancing your loved one’s needs with children, work, and your own life.
You don’t need a complete plan before reaching out. We help you find a starting point.

A professional to help carry the load
You shouldn’t have to become an expert in aging care overnight. We help organize the information, clarify your options, and coordinate with the people involved in your loved one’s care.
Your loved one receives respectful, personal attention. Your family has a steady point of contact to help track next steps, prepare questions, and follow through—so every detail doesn’t have to live in your head.
We work alongside your loved one’s licensed providers. They make medical decisions; we help your family navigate the process and put the plan into motion.

What you receive
Your toolkit reflects the scope we agree on. These brief examples use entirely fictional information.
A clear timeline: Oct. 2 — discharge summary lists follow-up within seven days. Oct. 4 — family reports no appointment confirmation. Open question: has the practice received the referral?
An action plan: Request discharge records — CCC — Tuesday. Confirm transport — authorized family contact — Wednesday. Medication-list discrepancy — question routed to prescriber; awaiting reply.
A community comparison: Compare fictional Options A and B by quoted cost, location, services, accessibility, and questions still needing answers. Availability and suitability must be confirmed with each provider.
Getting started: 1. Meet for a free 15-minute introduction. 2. Agree on deliverables, limits, timing, and fees in writing. 3. Sign the client agreement and pay before paid work begins.
Records review, provider outreach, written plans, and ongoing coordination are paid services. Medical, legal, and financial decisions remain with you and the appropriate licensed professionals.

A stack of unopened mail. Scattered medical records. Forms nobody has explained. Appointments, bills, and decisions that keep coming.
Charleston Care Collective helps turn that overwhelm into an organized system—with clear priorities, practical follow-through, and someone keeping track of the details.
From incoming mail to next steps.
With your permission, we set up workflows to scan and organize incoming mail and email, identify items needing attention, and turn deadlines into tracked tasks and reminders. Automation supports our review and follow-up.
Less paperwork chasing.
We help submit your existing power of attorney and required authorizations to banks, hospitals, insurers, and other organizations you identify. We track receipt, additional forms, and outstanding requests to reduce avoidable trips and repeated calls.
Signing support that comes to you.
When notarization is needed, we can coordinate an independent mobile notary in the Charleston area, subject to availability.
A medical history you can actually follow.
With appropriate authorization, we request records and organize hospitalizations, visits, documented medication changes, vital signs, and progress notes into a concise timeline. We review it with you and distinguish what the records say from what your family observed.
We also help you document changes you noticed at home and prepare sensitive questions or family observations to share with the treating team—so important context has a place in the conversation.
A clearer picture of accounts and bills.
We help you or your authorized representative request credit reports and organize them alongside statements, invoices, and incoming mail to build an account inventory and track bills requiring attention.
Medication information in one place.
We organize provider-issued medication lists, pharmacy contacts, and refill dates, and help route questions or conflicting information to the pharmacist or prescriber.
Community options shaped around your loved one.
We work with independent community partners to identify options reflecting your loved one’s preferences, budget, location, and care needs as identified by their providers. We help compare options, arrange tours, and accompany you when requested.
Follow-through you can see.
A shared calendar and action list show what’s completed, what’s waiting on someone else, and what needs your decision next.
Services are tailored to your agreed scope and authorizations. Professional services and third-party costs are confirmed separately.

The right people. Someone connecting the pieces.
Your loved one may need several kinds of help. Finding the right professionals—and keeping everyone informed—can become a job of its own.
We help your family navigate relevant medical, mental health, caregiving, legal, and community resources. We coordinate with existing providers and help you connect with additional expertise when needed.
Independent professionals provide their own services under separate agreements with your family. Charleston Care Collective provides the navigation, organization, and follow-through that help those services work together.
